4 Small-Cap Stocks with Catalysts Investors Can’t Ignore

Investors continue to focus heavily on the industry's giants, including Novartis (NYSE:NVS), Eli Lilly (NYSE:LLY), and Amgen (NASDAQ:AMGN), but the latest market action is a reminder that some of the biggest percentage moves in biotechnology can come from much smaller companies.

This week, Tenon announced that it repaid its approximately $5.16 million of outstanding original-issue-discount senior convertible notes ahead of their September 11 maturity date. The notes were issued in March and had the potential to convert into common shares at a discount to market prices.

Tenon's repayment removes that particular overhang and, after a hard pullback, Tenon appears to be pile driving the shorts. CEO Steven M. Foster said: “By proactively addressing this obligation, we are reducing potential dilution for our shareholders, strengthening our financial position and maintaining greater flexibility to invest in the continued commercialization of our products and expansion of our business.”

The company's second-quarter 2026 numbers offered an important signal. Tenon reported revenue of approximately $1.3 million, up 127% from the year-earlier period, while gross profit increased 232% to roughly $0.8 million. The company also reported a 64% gross margin. Management said case volume was growing across both the Catamaran and SImmetry+ platforms.

Foster described the quarter in direct terms: “Revenue of $1.3 million and gross profit of $0.8 million were each the highest we have reported in a second quarter.” He also pointed to improving economics as procedure volume expands, suggesting that the investment case increasingly revolves around whether Tenon can translate growing utilization into a sustainable commercial model.

Tenon completed a 1-for-35 reverse stock split in August and has been working to regain compliance with Nasdaq listing requirements. The company also disclosed significant financing needs, including convertible notes coming due and subsequent capital-raising activity. That creates a classic small-cap setup: commercial momentum is encouraging, but capital structure, liquidity and Nasdaq compliance remain critical variables. Tenon may have a compelling operational story, but investors should recognize that execution risk extends well beyond the operating room.

NeOnc Technologies Holdings: A Clinical Catalyst Has Arrived

NeOnc (NASDAQ:NTHI) may be the most immediately event-driven name on this list. The company's lead program, NEO100, is an intranasally administered formulation of purified perillyl alcohol being developed for central nervous system cancers, including recurrent IDH1-mutant high-grade glioma. The company's underlying thesis is that intranasal delivery may help address the blood-brain barrier, one of the major obstacles in treating brain tumors.

That thesis received a major test in August when NeOnc announced positive topline Phase 2a results. The company reported six-month progression-free survival of 48.9%, compared with a pre-specified 20% benchmark, with a reported p-value of 0.0047. It also reported median overall survival of 26.09 months and said 86.7% of patients were alive at six months.

CEO Amir Heshmatpour had previously described the Phase 2a readout as one of the company's most important clinical milestones. Following the data, NeOnc said it planned to request a Type B meeting with the FDA to discuss a potential registrational path. The company is also developing NEO212, an oral conjugate involving temozolomide and perillyl alcohol, adding another potential clinical catalyst to the story.

This week, NeOnc announced a $15 million registered direct offering, at $4.20 per share, involving common shares or pre-funded warrants and accompanying warrants. That financing strengthens the company's capital position, but it also demonstrates the fundamental tension in clinical-stage biotechnology: promising data can create opportunity, while advancing programs requires substantial additional capital.

Opus Genetics: September Could Bring a Major Gene-Therapy Test

Opus Genetics (NASDAQ:IRD) is taking aim at one of biotechnology's most ambitious areas: gene therapy for inherited retinal diseases. Its pipeline contains multiple AAV-based programs designed to address genetically defined causes of severe vision loss, including OPGx-LCA5 and OPGx-BEST1, with additional programs targeting RDH12, MERTK and RHO.

The immediate attraction is timing. Opus said in August that three-month topline data from Cohort 1 of the OPGx-BEST1 Phase 1/2 study were expected during the second week of September 2026. The company also reported that enrollment had been completed in its LCA5 registrational study, with dosing expected to begin during the fourth quarter. In other words, investors are approaching a period in which several pieces of the pipeline could begin producing meaningful evidence.

CEO George Magrath framed the opportunity around execution, saying: “We are rapidly advancing five gene therapy programs that address significant unmet need in inherited retinal diseases.” Management has also highlighted a cash runway expected to extend into 2029, a potentially important advantage for a development-stage company attempting to advance several programs simultaneously.

Gene therapy development can involve complex manufacturing, regulatory and clinical challenges, and positive early-stage data do not guarantee registrational success or commercial approval. Still, the combination of multiple programs, a potentially important near-term BEST1 readout and additional clinical initiations makes IRD one of the more event-rich smaller biotech names entering the fall.

bioAffinity Technologies: From Biotech Story to Commercialization Story

bioAffinity (NASDAQ:BIAF) offers perhaps the most differentiated setup among the four smaller companies because its lead opportunity is not simply a drug candidate. The company is commercializing CyPath Lung, a noninvasive diagnostic designed to assist in the detection of lung cancer, while simultaneously developing additional diagnostic and therapeutic programs.

The company's second-quarter 2026 update provided a notable commercial data point: CyPath Lung test volume increased 216% year over year. bioAffinity also reported expanding physician adoption and said it was advancing a longitudinal clinical trial. In August, the company announced a nationwide federal healthcare distribution agreement with AvMEDICAL, adding another potential avenue for broader commercialization.

CEO Maria Zannes emphasized the commercial side of the story, saying: “During the second quarter, CyPath Lung test volume surged 216% year-over-year.” That growth matters because diagnostic companies ultimately need to demonstrate that physicians, healthcare systems and patients will actually use their products—not merely that the underlying technology is scientifically interesting.

bioAffinity completed financing during 2026 and enacted a 1-for-15 reverse stock split in August. The company subsequently announced on September 9 that it regained compliance with Nasdaq listing requirements, removing one immediate exchange-related overhang. With commercial test volumes rising, a federal distribution relationship in place and additional diagnostic programs targeting asthma and COPD, BIAF is increasingly a commercialization story rather than merely a laboratory-stage biotech narrative.
