Rapport Therapeutics Scores Bipolar Mania Breakthrough as RAP-219 Advances Toward Phase 3

DENVER, Colo. (247marketnews.com) -- Rapport Therapeutics (NASDAQ:RAPP) delivered a major clinical milestone, announcing positive Phase 2 results for RAP-219 in adults experiencing acute bipolar mania. The randomized, double-blind, placebo-controlled trial enrolled 253 adults with bipolar I disorder and met both its primary and secondary endpoints, strengthening the case for a potential new treatment in a field where tolerability remains a significant challenge. With additional Phase 3 programs already underway in epilepsy, Rapport is building a broader development story around a single investigational drug with multiple potential applications.

At Week 3, RAP-219 produced a statistically significant 3.1-point improvement over placebo on the Young Mania Rating Scale (YMRS), with patients receiving treatment showing a 15.4-point mean reduction from baseline versus 12.3 points for placebo (p=0.045). The drug also achieved a statistically significant improvement on the prespecified Clinical Global Impression–Bipolar Version mania severity endpoint (p=0.024). All 11 YMRS items improved numerically relative to placebo, including measures of irritability, thought content, disruptive-aggressive behavior, and sexual interest.

Safety and tolerability could be important differentiators if the findings hold up. During the three-week treatment period, adverse events occurred at similar rates in the RAP-219 and placebo groups, and all reported treatment-emergent adverse events were mild or moderate. No serious adverse events were observed during that period. Agitation was the most common adverse event in the treatment group, and 3.2% of RAP-219 recipients discontinued because of an adverse event, compared with 0.8% receiving placebo. Rapport CEO Abraham N. Ceesay emphasized the importance of tolerability, stating, “For people with bipolar disorder, the challenge isn’t just finding a treatment that’s efficacious. It’s finding one they can tolerate and stay on.” The eight-week post-treatment safety data have yet to be presented, so the longer-term profile remains an important question.

The next catalysts are already taking shape. Rapport expects to meet with the FDA in the first quarter of 2027 to discuss the bipolar mania results and Phase 3 trial design, with two pivotal trials anticipated to begin in the second half of 2027. Meanwhile, its FOCUS 1 and FOCUS 2 Phase 3 trials of RAP-219 for focal onset seizures are enrolling, and a Phase 3 study in primary generalized tonic-clonic seizures is expected to begin in the first half of 2027. The company reported $436.1 million in cash, cash equivalents, and short-term investments as of June 30, 2026, excluding restricted cash, with funding expected to last into the second half of 2029. That financial runway provides meaningful support for its development strategy, although trial costs and execution risks remain.
